During the enrollment process, you will receive and acknowledge receipt of additional notices and disclosures
You will receive a full copy of your policy from the insurance company if your enrollment is approved. It is your responsibility to confirm that you have received policy documents and that you have paid the invoice from the insurance company. If you have not received policy documents, or have not paid the invoice from the insurance company, then you do not have coverage. Call LTC Solutions, Inc. immediately at (877) 286-2852.

Increases to existing coverage are allowed at open enrollment.
Visit the Apply Now section of this website and follow the instructions to access the online application and complete a new application subject to full underwriting. New policy will stack on top of the existing policy.
Decreases to existing coverage are allowed at anytime.

Cancellations are allowed at anytime.

Complete the Request Duplicate Certificate Section of the Request for Service Form and submit via secure file upload here.
Complete the Tobacco or Nicotine Use Questionnaire and submit via secure file upload here.
Complete the Beneficiary Change Form and submit via secure file upload here.
Complete the Address Change Section of the Request for Service Form and submit via secure file upload here.
Complete the Name Change Section of the Request for Service Form and submit via secure file upload here.
For use by policyholders who are currently on direct billing via monthly automatic payment with Chubb and need to make one of the following changes.
- Change bank account
- Need to correct bank account
- Change the date of their withdrawal
