*Benefit eligible employees working 30+ hours per week are eligible to apply. Participation requirement of 5% approved employee applications needed to offer Guarantee Issue. All employees will answer underwriting questions in their application – if participation requirement is met, application will be processed with Guarantee Issue and the underwriting questions will be disregarded. Guarantee Issue up to $150,000 for employees ages 19-70. Other policy options are available with Underwriting consideration. Spouses and domestic partners require Underwriting consideration for all levels of coverage.
